What changes at 65

Medicare is new. Your health history goes back decades.

Sixty-five brings a new card and a new set of rules. Some people keep their doctors. Others change plans, and their doctors change too.

Either way, your history is spread across decades of offices. Some have closed or merged, and many use systems that don’t share with each other. When a new doctor asks about your history, you’re the one expected to remember it.

What Milepost is, and isn’t. It keeps your health history ready for your doctors. It doesn’t compare, sell or recommend Medicare plans.

How it works

Three steps, done once, kept current

  1. Start your trial

Choose whether the profile is for you or for a parent you look after, tell Milepost where you live, and start your 7-day free trial.

  2. Confirm it’s you, and meet a clinician

Enter your details, verify your identity and address, choose who can see your profile, sign the consent forms and schedule an intake visit with a clinician.

  3. Your records come to you

Triangle requests your records from doctors and hospitals through the national networks, Carequality and TEFCA. Upload anything you have on paper, add a family member to help if you like, then read your summary.

What you get

Your health history, gathered and kept in order

  1. No more retelling it from memory

Records from the doctors and hospitals you’ve seen, in one current history, updated as new records arrive.

  2. A summary for any new doctor

Your medical history in words you can follow, ready for a new doctor or a wellness visit.

  3. Your medicines and labs, in order

Medicines listed from your records, including pharmacy fills when your records contain them, and results like cholesterol, blood sugar and kidney function as trends with plain explanations.

  4. Answers from your own records

A chat that knows your history. A second check reviews answers against the evidence before you rely on them.

  5. A clinician who has read your history

Membership includes a review of your medical history by a licensed clinician, and setup includes an intake visit with one.

  6. Help from family, on your terms

Share your profile and choose who can see it and who can help manage it.

Your own assistant

Use Claude, ChatGPT or Grok with your history

If you already ask an AI assistant questions, you can connect it to Milepost. With your permission, it can read your records, medicines and lab results, and answer from them.

Questions you might ask it

  • “What should I mention at my Welcome to Medicare visit, based on my records?”
  • “Read me my medicines and doses, so I can tell the new doctor’s office.”
  • “How has my A1c changed over the last three years?”

In plain wordsOnce you connect an outside assistant, what it reads is handled under that assistant’s own privacy policy, which Triangle does not control. Connect only an assistant you trust. It works only with what’s in Triangle and can’t change anything at your doctor’s office. You can disconnect it any time in Triangle under Settings → AI service connections, and access stops right away.

For familiesOnly the person whose profile it is can connect an assistant. If your mother uses ChatGPT, she can connect it to her own profile. A family member who helps manage her profile can’t connect one for her.

Questions

Answers before you begin

Will Milepost help me choose a Medicare plan?

No. Triangle doesn’t compare, sell or recommend plans. For help choosing, use the plan finder on Medicare.gov, or talk to your State Health Insurance Assistance Program (SHIP), which Medicare.gov lists by state.

What does it cost?

Milepost starts with a 7-day free trial. You add a card when you start, and if you cancel before day 7 you’re not charged. After that it’s $9 a month, and you can cancel any month. Your records stay yours.

Membership covers Triangle requesting your records from your doctors and hospitals and keeping them updated as new records arrive, chat any time with the assistant that answers from your own records, a review of your medical history by a licensed clinician, connecting wearables such as a Fitbit or Garmin, and uploading anything else you have.

What does setup involve, and can my daughter or son help?

After you start your trial, you’ll tell Milepost where you live, enter your details, verify your identity and confirm your address, choose who can see your profile, sign the consent forms and schedule an intake visit with a clinician. Then you can upload anything you have on paper.

Family can help. You can add a family supporter, share your profile and choose who can see it and who can help manage it. A son or daughter can also run a profile for a parent they look after.

Will it reach doctors I saw years ago?

Triangle requests your records through the national health-record networks, Carequality and TEFCA, and a clinician signs the request with you. Not every office can be reached that way. Upload anything you have on paper, and Milepost shows what may be missing and helps you request it.

Can it connect to my Medicare account?

Not today. Triangle gets your records from your doctors and hospitals through the national health-record networks, and you can upload anything you have on paper.

Does Milepost diagnose, prescribe or replace my doctor?

No. Triangle provides research and educational support. It helps you understand your records and prepare questions for your doctors. It does not diagnose, treat or prescribe, and it never replaces your care team.

Is my information safe?

Everything is encrypted, and your records are yours to delete. Your data is never sold or shared for advertising. Triangle is HIPAA compliant, and its practices are in its Trust Center.
